At Elmhurst Extended Care Center you will find a small and cozy atmosphere where we are dedicated to our residents and their families.
Our nursing and care staff is excellent and we are proud to say our staff-per-resident ratio is one of the highest in the State of Illinois. In fact, we received a special commendation from CARF (Commission on Accreditation of Rehabilitation Facilities) for our experienced and caring staff.
Our experience in rehabilitation, nursing care and Alzheimer’s care has been well established. We are the oldest privately held nursing care center in DuPage County and one of the few family managed and owned skilled care centers in the State.
Over 80 credentialed physicians see their patients here and recommend us as a gem of good care in the community.
Elmhurst Extended Care Center is Medicare and Medicaid certified and offers long-term skilled care and certified Alzheimer’s care at rates far below other nursing care centers, resulting in a real cost savings for our residents and their families.

REHAB
Elmhurst Extended Care Center prides itself on the quality of our rehabilitation program. With over 40 years of experience, our in-house therapy team works with you and your doctor on a recovery plan to help you regain strength and independence.
Our short-term rehabilitation is Medicare certified and includes physical, occupational and speech therapies after a hospital stay.

Right from the beginning, our Rehab/Care Plan Team works with you and your family to find resources in the community that will help you continue your recovery at home.
NURSING CARE
We’re family oriented and it shows in the exceptional care we provide our seniors. Around the clock nursing care is given in a home-like environment where each resident is treated with kindness and respect.
At Elmhurst Extended Care Center, we not only promote physical healing and restoration, we cater to each individual’s social, recreational and spiritual needs. Our top priority is to help each resident be as independent and fulfilled as possible.
Nursing services include:
- Individualized Care Plans by an interdisciplinary team of physicians, nurses, therapists, dietitian and social worker
- Restorative Care and Fall Reduction
- IV Administration
- Blood Sugar Monitoring
- Wound Care
- Catheter, Colostomy, Tracheostomy, and G Tube Care
Delicious meals are prepared and served each day in our lovely dining rooms. Personal choices are offered and therapeutic diets are carefully overseen by our registered dietitian.
ALZHEIMER'S CARE
We understand that choosing care for your loved one with Alzheimer’s or other memory related conditions is stressful and difficult for families. We want to help.
With a recommendation from your doctor and an interview with our staff, we help to create a specific plan of person-centered care. Our activities program focuses on individual abilities, and is adapted to help each resident reach their highest potential and participate in their favorite life activities.
From one-on-one interactions to group activities, our residents are treated with understanding and respect. Often residents enjoy just reminiscing about family and fun times past.
Our Certified Alzheimer’s Unit is located on the 2nd Floor of Elmhurst Extended Care Center and is alarmed to maintain safety for our residents. The staff receives specialized training to meet the challenging requirements of caring for our residents with memory loss behaviors.
We understand that families need support as well. Our Unit Nurse Manager, Social Worker, Activity Director and Patient Care Plan Coordinator are key personnel that are happy to help with questions or concerns. Elmhurst Extended Care Center also offers monthly Alzheimer’s/Dementia support group meetings held on the last Monday of each month.
PERSONAL SERVICES
Elmhurst Extended Care Center offers a variety of personal services that help our residents feel at home. From individually appointed rooms to beautifully decorated recreational and social areas, we take pride in the warmth of our Care Center.
Activities are available seven days a week and include an array of recreational, social, and spiritual programs focusing on the wishes of our residents and their families. We also have a handicap accessible bus so residents can enjoy field trips and outings into the community.
