Stephenson Nursing Center is a not-for-profit long-term care facility owned and operated by the people of Stephenson County.
The facility is licensed by the Illinois Department of Public Health for both skilled and intermediate levels of care, and is certified for both Medicaid and Medicare.
The facility is located south of Freeport in a rural setting on the grounds of the original county poor farm. In keeping with the philosophy of its origin, admissions of Stephenson County Residents take priority.
Residents are admitted without regard to race, creed, color, national origin, religion, gender, handicap, or ability to pay.

STEPHENSON NURSING CENTER'S MISSION
We strive to provide an environment as homelike as possible and to offer care and services to maintain each individual at his/her highest level of functioning. We strive to meet each Resident's individual physical, mental, and psychosocial needs and strongly encourage family involvement.

ABOUT THE FACILITY
The center has 160 beds. There are five private rooms and three special care rooms. All other rooms are semi-private with a shared bath. Residents are encouraged to personalize rooms within the space limits.
There are four nursing units, each with a dining and lounge area in the one story facility. They are divided into level of care including Skilled, Intermediate Care, and Limited Assistance.
Our nursing unit with 24 beds specializes in the care of Residents with Alzheimer's disease or related disorders. The unit has a staff of caring individuals who have specialized training in the care and treatment of these Residents. The unit has been designed to limit distractions, allow for freedom of movement, and has a beautiful fenced area for outside activity.
The staff of 170 provides services in order to meet the daily needs of the Residents.
