Willow Crest Nursing Pavilion offers loving care in a warm and homelike environment. This is our residents' home, and we encourage everyone to look at it that way. It's often the little things that make the difference, such as encouraging families and residents to personalize their rooms with familiar belongs-from furniture to photographs-so they feel more comfortable and at home.

Our goal is to provide accurate, individualized, medical attention to every resident. Each resident is entitled to live the fullest life possible consistent with his or her own physical, emotional, and intellectual capabilities.

Our services include:
Respite, short-term, and long-term care
Alzheimer's disease and dementia care
Holistic Personal Care Plans

* Initial screening, evaluation, and assessment by cross-functional team and new resident and/or family
* Customized plan to meet each resident's individual needs
* Nursing, dietary, social services, psychosocial, therapies, and activities
* Joint quarterly reviews with resident, family, and staff

Skilled nursing professionals 24-hours per day

* Registered nurses (RN)
* Licensed practical nurses (LPN)
* Certified nursing assistants (CNA)

Restorative Nursing Programs

* Help resident achieve the highest level of functioning in bathing, dressing, grooming, eating, and mobility
* Staff follows individual written plan developed by restorative nurse

Rehabilitation Programs

* Physical, occupational, and speech therapy based on physician's orders and resident's tolerance
* Licensed therapists
* Wound care
* IV therapy and pain management

Hospice Care
Social Services Program

* Emotional support to residents and families
* Referral services to community resources
* Monthly satisfaction interviews
* Arrange hearing, dental, optometry, and podiatry appointments

Activities Program

* One-on-one, small, and large group activities and events
* Inclusive to all residents
* Friends and family are encouraged to participate
* Inter-generational programs include children, animals, music, art, education, exercise
* Weekly excursions to shopping, restaurants, museums, culture events, and historical sites
* Library books on tape
* Faith-based and non-denominational religious services and programs
