Welcome to Fondulac Rehabilitation & Health Care Center,
On behalf of the residents and staff of Fondulac Rehabilitation & Health Care Center, I would like to extend a very warm welcome to you and your loved one. The staff of Fondulac Rehabilitation & Health Care Center strives for excellence in obtaining each resident’s individual needs and goals to attain their highest level of independence.
Our main objective is to provide excellent nursing care. We have a very dedicated, caring and professional staff to meet your needs. We also offer an exciting activity and social service department to provide for your social needs. We strongly encourage family involvement with our residents at any time. We have multiple community and family events throughout the year.
Please excuse our mess, we are currently remodeling the entire facility. We are in the middle of painting, recarpeting and tiling. The construction has given Fondulac a fresh new look. If it has been awhile since you have seen the home, we would love to give you a tour.
SERVICES OFFERED
For Potential Residents -
- We have 98 dully certified Medicare/Medicaid beds, which can also be paid for Privatly...
- A benifit over most of our competitiors in the fact if Medicare days have been exhausted, you are welcome to stay as long as you like! No moving, no changes, no hassle!
- We have both private and semi-private rooms available to our residents.
- We accept Medicaid Pending Residents.
- We provide top of the line, in-house therapy services by Rehab Care...
- Services such as Physical Therapy, Occupational Therapy and Speech Therapy are offered 7-days a week by a caring, compasionate staff of professionals.
- Certified Dietary Manager and Registered Dietician on staff. Thus, making us able to handle any of your dietary needs.
- A wonderful Housekeeping and Laundry staff attends to the overall cleanliness of our resident rooms and the laundry needs of our residents.
- 24-Hour nursing care capable of managing IV Antibitoics, Trach Care and Wound Care Management in a safe and secured atmosphere.
- Our Medical Director, Dr. Greg Johnson, is on call 24-hours a day and is always willing to answer any family questions or concerns.
- Complimentary cable television is accessible to all resident rooms.
- Wireless Internet is available at no charge throughout the facility.
- Semi-private bathrooms with a maximum of 4 residents using the same facilites.
- A wonderful activities program including all of your favorite games and activities!
- Resident banking is available to those who wish to have the facility manage their money in an interest baring account. Of course, residents frequently have opportunities to both deposit and withdraw monies from their account.

RATES
Fondulac Rehab and Health Care is proud to offer our residents multiple payment methods for their services recieved. With 98 dully certified Medicare and Medicaid beds, we are able to make Medicare and Medicaid viable payment options for those who qualify.
Another payment option is privately paying for services at Fondulac Rehab and Health Care. As of March 15, 2008 - our current rates are as follows:
Semi-Private Room - $123.00/day
Private Room - $140.00/day

PROGRAMS
Fondulac Rehabilitation & Health Care Center offers a variety of individualized care programs and medical services to help residents regain function and get the most out of life.
NURSING SERVICES
Our Nursing Team consists of compassionate, well-educated professionals whose skills and experience are focused on meeting the total care needs of our residents! Our nursing staff is continually updated and trained on the most progressive treatment options available anywhere. They are on duty 24 hours a day, 7 days a week to serve our residents and answer your questions!
DIETARY
Our dietary specialists know the value of a well-prepared, appetizing meal. They are uniquely equipped to meet the specialized dietary needs of our residents as well as accommodating a wide variety of personal preferences shaped by a lifetime of experience. Friends and family members are encouraged to join us for delicious meals on a regular basis. There is a small charge ($2.00) for those wishing to join us.
SOCIAL SERVICES/ ACTIVITIES
Our social service department in conjunction with our interdisciplinary team generates quality programs whose purpose is to fulfill the varied needs of our residents. The goal of our activities team is to provide a dynamic and stimulating home environment. Both departments provide challenging individual and group programming designed to promote intellectual, physical and social stimulation and rehabilitation.
